This achievement is story related and unmissable.
Proceed through the first level. Once you reach Azai's room a cut scene will trigger. When the scene ends, kill the two guards in the room. The achievement should then pop during the end level cut scene.

Story Related, cannot miss.
You must save Azai and 4 other Ninjas at the end of level 1. If you want to be stealthy, make your way from the top of the room with Azai, to the right and down a series of vents until you are under and behind the guy on the left side of the room with Azai. Stealth kill him and then the guy beating Azai. Once they're dead, hold 
when you're standing near Azai to free him.
