This achievement is story related, so you cannot miss it.
You have to complete Level 9. Killing Karajan is the last thing you have to do in this level.
To reach him you must lower the security systems (marked on the map). Once all are deactivated, head to the room with Karajan (also marked on the map) and kill him. There is no cover in this room so just walk to him and press X.

Story related, cannot miss. Level 9.
To reach Karajan, you need to lower the security systems of the castle that are keeping you from reaching him. Once that is done, head to the room with Karajan (marked on the map) take him out as you would anyone else.
