Very simple achievement.
As the description says, you need to use your equipment to distract the guards, meaning, your Distraction Items such as the Noise Maker. Simply use the Noise Maker to distract 25 guards and perhaps the dogs* are included as well. I'm not sure if it counted towards the achievement, but you can also distract the dogs.
This achievement does not require you to do it all in one level as achievements like Trickster do. I don't even think you can since you can only carry 4 Noise Makers, so just play through levels distracting guards (with your distraction items, of course) until you unlock the achievement.
*EDIT: BoatManJuzz07 has confirmed that the dogs in fact count towards the achievement as well.

Just to confirm that dogs do count. I distracted a dog that was alone and the cheevo popped

Much of your progress toward this achievement will come from using the Noisemaker item you receive early on in the game. Select it using the dpad, and the pull the 
to aim within earshot of an enemy, and tap 
to toss it. To make it go by quicker, you can just distract groups of enemies, and/or keep reloading at a checkpoint with a group that is easy to distract.
