The hovering enemies will spit out homing flameballs later in the level. You need to lure these so it hits a box. I found after checkpoint three, once you are past the red ring, there is a flameball that can be directed towards a heart box.

This is a weird one. I tried it on multiple other boxes throughout other levels but the achievement would only unlock when I destroyed the specific one mentioned in this guide. I destroyed other heart boxes so I know I was doing the correct ones. Just thought I'd save someone else some time!
