Once you have the tarot cards, you need to use them on 4 seperate chapters. Select them in your inventory, shuffle, select divination and pick your cards. It does not matter which cards you pick, you will always get the same ones.

As soon as you get the quest to find the Tarot cards, go upstairs to your parents bedroom and in one of the drawers in the desk are the cards.
Every new chapter when you get out of bed you need to hit R1 and bring the bag up and select the deck and then shuffle and perform a divination.
The trophy will unlock during the chapter "The Graveyard" if you've done it correctly.
