Really simple. As you fight through the campaign mode, you will unlock costumes for characters after beating certain stages. The next time you pick that character in a versus battle, simply select an alternate costume for them, and this achievement will unlock.
I believe the earliest possible one, if your following a straight forward path in the campaign is Iron Man's movie costume.

Your correct on the latter point. As for the question, not sure. I also use the controller, but unless if your Kinect is picking up last minute body movements indicating you want to change back, not sure.

Odd. I can't remember if I actually carried the costume through. I popped the achievement, and honestly didn't care after that. : P

Costumes can either be unlocked via Campaign, challenges, ranking up, or bought from the marketplace. After choosing your character, simply hover over them and slide your hand to change their costume. The achievement should pop immediately afterwards.
