The Fully Loaded achievement in Marvel Puzzle Quest: Dark Reign worth 361 pointsCollect 15 covers for any character.
It is the 1st level called "The Bench" You will be fighting 2 lvl 1 thugs.
Usually the map starts out with a lot of red on the map other then that the cascade is pretty much luck based.
This place is a good place to grind since the team up tiles have not been introduced yet so more chance of getting cascade matches.

This is how I got it. This one is really luck-based but you should be able to get it pretty easily though at some point over the course of your playtime.

Similar to "The Ocho" achievement, this should come just through normal game play going through the story mode. With the game boards being randomly generated and adding in new random tiles as others are matched and used up, there's a chance that tiles will drop into place and create a new match after one has just been made, thus making a "combo". This achievement therefore has a lot of luck to it, so just keep playing and this should pop eventually well before you reach the end of the story mode. If you're unlucky to not get this, then try replaying early chapter battles with lower level characters to extend fights, giving more chance for matches and combos.
