UPDATE: WITH THE LATEST PATCH (10th of december) THIS ACHIEVEMENT IS CURRENTLY UNOBTAINABLE. DEVS SAID THAT ARE GOING TO REVERT IT IN A FUTURE PATCH
(A bit more of info, due to a bug where Dr Strange Ulti would charge up when ALLIES would die in his portal, they now set the portal kill as a suicide instead, voiding the achievement. Devs said as soon as they'll fix the bug, the achievement should become obtainable again)

I just wanted to add one more solution compared to the one of MrWebHead (which works!)

In the game mode where a team attack and one defend, if you are into the DEFEND team you are allowed to roam the map before the enemy team does, hence you can put a portal STRAIGHT UP in front of their spawn, that of course leads to their doom (outside the map).
It only works (almost guaranteed) in the very first moment of the game, when players rush outside the door, I did a few double and triple kills like that (cheesy but I'll take it)

As people become more aware of the game mechanics, the solution with placing the portal in front of the spawn point might become incredibly hard. They just won't walk into it like before.

A solution I found that works is if you can pair with a friendly Jeff, they can ult and spit enemies into your portal, crediting you with the terrain KO. This might become a little more tricky as season 1 approaches and Jeff's ult hitbox is becoming a little more restrictive.

Here is video proof from my POV as Dr. Strange, when my friend spits an enemy into my portal. Plus a surprise of an enemy naturally jumping in.

2) I NEED CONFIRMATION FOR THIS METHOD, but I'm fairly sure it works.
The achievement states 1 terrain KO, NOT AN ENEMY, hence you can ask a friend to jump into the portal and he/she can die for you. I unlocked that way the first time, but I'd like further confirmation if that was a coincidence or not. I'll edit the guide if it doesn't work.
