Hello. I'd thought I'd share what it took for myself and a couple of other achievement hunters on how to pop this achievement.
In order to pop this achievement, we did 3 KO's each for three methods.
1. Killing the opponent with the grenade itself.
2. Killing the Opponent through the smoke with the Assault Rifle.
3. Killing the opponent through the smoke with the Shotgun.
We came to the conclusion that the achievement is triggered when you kill someone the moment they are encased in the smoke grenade. This means you need to shoot the grenade onto the enemy (don't KO them with it) and then immediately shoot them with either weapon.
It definitely is an annoying one to get (Took me hours of attempts while others were unlocking it). Hopefully this helps.

Thats when it didn't pop when I did only with Smoke greande killed 3 same time. It popped doing 1 by 1 kills 3 times with finishing them off with smg during the smoke, finishing them off 3 times with shotgun during smoke and the 3 Smoke grenade kills

I got this one rather easily. The best advice I can give is to get them really low with the shotgun and then throw the grenade. If they are low enough the grenade itself will kill the enemy.
