This achievement seems bugged or the requirements are not clear. In two separate games, I have landed 3 and 4 player KOs with Squirrel Girl's Ultimate but have not unlocked this achievement. Looking at this achievement on Steam, it also has a 0% unlock rate while every other character achievement has at least .1%, so I wouldn't go for this achievement for the time being.

Someone on psnprofiles says you have to get the 3 kills with the first hit of the wave, if it bounces off a wall any kill after wont count.

It seems the description is wrong and you have to get three KOs on the initial launch of the ult meaning before the squirrels bounce at all. If they bounce and you get KOs after that, it won’t count.

This achievement is terribly tricky and doesn’t give the best description.
The guidelines to get this achievement, as far as I’ve seen it, is this.
-When you use the ult, it must hit 3+ enemies and those enemies MUST be killed before the squirrels bounce off any wall or structure
-You DON’T need to get the finishing blow on every enemy, as long as you hit them with the ultimate, any teammate can assist you on killing them and you can attack after the ult too.
-This can be done in both Quick Play and Conquest. I wouldn’t advise Competitive for obvious reasons.
-Any achievement CANNOT be done in Practice Range or Against AI
Now this is what I did to get it and seems the best and easiest way to get it.
It involves playing Conquest with 2 people, one as Squirrel Girl and the other as Groot.
You play normally until both of you get your ultimates. Then you must lure 3+ enemies near the corner of the map (the corner of the squared road). Once the enemies are together, use Groot’s ult to pull them all in together as it keeps them together for only a few seconds. Right after Groot ults, Squirrel Girl must ult and hit all 3+ enemies. As soon as those enemies are hit, you have to hurry and attack them quickly to kill all 3+ enemies before the squirrels hit a wall. This is also why you must be near a corner of the map so you’ll have more time to kill them before they hit a wall.
It’s worked well as both me and the other person got the achievement!
