For this and Mantis achievement you actually have to get the assist on all deaths. I know some people were saying they hit the opposing players before their teammates killed them but if game doesn't register an assist for you it won't count. Hopefully that clarifies it for you

The other guides are a little sparse and not entirely correct, and I finally unlocked this today so I'll explain the actual requirements and provide some tips here.

First off, you DO NOT have to assist all six KOs in a team wipe, but you DO have to participate in all six KOs. In essence, in order for a KO to count toward this achievement, you need to see either a Final Hit, KO, or Assist notification pop up on your screen when a KO happens. Secondly, this is incredibly RNG-based, so you'll either get incredibly lucky and obtain it in the first few games, or like me, this will take forever and you'll need the perfect combination of skill/team skill/being in the right place at the right time and it helps tremendously if the other team is bad and also not so bad that they don't stay grouped up - if it's a bad team that just rushes in individually and dies, you likely won't even be able to get a team wipe because the respawn timers are so short. Don't even attempt this if the other team has Hulk or Adam Warlock. By the time you've whittled Hulk's health down, one of your opponents will have respawned before his transformation into Bruce Banner has completed, and same goes for Adam Warlock and his cocooning.

A team wipe occurs when all enemy players are KOed before any single one of them respawns. You will see the word "ACE" pop up on the screen when this happens.

To sum up the requirements, if you participated in all six KOs, either by getting the final blow, dealing significant enough damage, or assisting a teammate by healing them, and see "ACE" on the screen, you will obtain this achievement at the end of the match.

Now, for some tips. First, this is a frustrating achievement. This took me a total of 13.8 hours of gameplay using Cloak & Dagger alone. 96 matches played, 53 wins, 1452 KOs, 1395 assists, and 4 team wipes before the achievement unlocked. For the first three team wipes, there was only one KO I missed out on, and in the third team wipe, I got the final hit on 5 players myself. If you are as unlucky as I was, try to just breathe through it, stay patient, and be grateful that Cloak & Dagger is a fun character to use.

The best advice I can give you is to try to always be in a position where you can see as many of your allies and as many of the enemies as possible, without putting yourself out in the open. Dagger's RT attack auto-aims, and if you're behind the fray and can also see most of it, you can just hold that RT button down and let it do most of the work for you. You'll likely pick up all the assists and KOs that happen that way just because it's so rare for one of Dagger's daggers to miss. If you get caught out in the open, or if an enemy diver hunts you down, use the LT healing bubble at your feet to help buy time for your own team to provide help.

Cloak's abilities are also incredibly useful if you can work out the timing of when to go on the offensive, so mastering the art of switching between Cloak & Dagger with RB is very helpful, and Cloak's LT ability also helps get out of danger in a pinch. Lastly, Cloak & Dagger's ultimate is a good way to initiate a team wipe if the enemy team is all bunched up. The team wipe that finally unlocked this for me happened because I used my ultimate to press behind enemy lines, switched to Cloak, took out their back line, then turned around and helped my team take their tanks out quickly.
