Similar to the "Undefeated Heroes" achievement, you can not revive any teammates or see the "Retry" screen when attempting this, but instead of just the boss fight, it is the entire level. If someone dies and you think you can complete the level without them, continue on. Otherwise, I'd restart the level.

To see which maps you have done this with, pause the game and go to Stats>Map. There are more than 25 maps in the game, so you can fail a few during the story and still get it in one playthrough without redoing any levels. If you fail on too many, you'll need to clean up the rest in the mission simulator.
