How to unlock the Very Elusive achievement in Mass Effect 2 - Definitive Guide
This achievement is unlocked during natural story progression.
After waking up in the Cyberus base and fighting along side Jacob as well as Wilson toward the hanger where Miranda is, you will then be taken to another Cyberus base were you will meet the Illusive man for the very firs time.
After completing the dialog with him this achievement will unlock.

This trophy is story related, and will unlock after you have completed your second mission, Prologue: Awakening. I will try and keep this as spoiler free as possible.
After waking on Lazarus station, you will be directed through several tutorials on how to shoot, melee, use biotic/tech powers, and apply medi-gel. This mission is very linear, and will cause you no problems. When you meet Jacob, you can earn another trophy here:
At the end of the first part you will meet Miranda, and you will then take a shuttle to another Cerberus location, Minuteman Station. When you regain control of Shepard, you can briefly talk with Jacob and Miranda, however both will direct you to speak with the Illusive Man. On the way to the comm room, you can earn another trophy here as well:
Upon entering the comm room, the Illusive Man will give Shepard his first mission - to investigate Freedom's Progress. Depending on your dialogue choices, you can earn some renegade/paragon points. After speaking with him, and leaving, the trophy will unlock.

another bugged trophy in this game, it does not unlock after the talk with illusive man
