How to unlock the Long Service Medal achievement in Mass Effect 3 - Definitive Guide
Fairly self-explanatory. You can either import one of your Mass Effect 2 saves from any time after the Suicide Mission, or play through Mass Effect 3 twice (with the second one either a straight-up New Game, or re-import that character using New Game Plus). Then complete the game. No different than in Mass Effect or Mass Effect 2.

If you have a Mass Effect 2 save file, you may import it to this game. Doing so will mean you only have to beat the game once. Choose Insanity to do it in one playthrough. If you don't have a save, then you will have to beat this game twice and start at Level 1 progress. Start on an easier playthrough since you'll have to beat it again anyways, then play Insanity on the 2nd.
Self-explanatory. If you have a Mass Effect 2 save (and again, I very highly recommend that you do), you only need to complete the game once. Without it, you’ll be starting a new game and need to play through it twice.

There is no difficulty at all in getting the trophy. if you have a save from the 2nd part, just transfer the character, go through the game and the trophy will fall out. If you don't have a save, just go through the game 2 times.

There is still a catch. The first time I finished, I shot at the Catalyst. By the way, with this ending they don’t give you the “legend” trophy. Going through it a second time, I chose the “blue” ending. And they didn’t give me a trophy. It dropped out after I loaded the last mission of the first playthrough, and in it I chose the “red” ending. Withdrawal, refusal to make a choice, or killing the Catalyst do not count as completion.

I want to share my personal experience.
I finished the game a couple of years ago, after which my old lady “veil” “decided to live a long time”, all saves were lost forever.
I bought a Slim and decided to play through the game again (I had just acquired parts 1 and 3, I wanted to play through the entire trilogy). There are no saves, as I already wrote. But the game saw the trophies from the account and, starting the game from the beginning, it gave out all the required bonuses (+25% to experience, credits, resources). I was happy, I thought that my passing would be counted as a repeat pass. But alas, no, for some reason this trophy did not drop. As luck would have it, it’s just not enough for me to reach platinum. You'll have to go through it again along the way.
