You can also send strike teams using the Apex HQ app on iOS and Android. ***WARNING*** I'm completely unsure how the app communicates with the Xbox as far as achievements. I imagine you have to at least login to your game for the achievements to pop, cuz they surely won't pop in the app. But I am unaware if you get no credit whatsoever for using the app. I unlocked the achievement prior the apps release.

It's looking as though completing missions within the app aren't being tracked for the achievement progression at this time.

Confirmation that someone got this achievement from using the app first, then accessing the strike team menus in game. Achievement having popped after about 30 seconds of accessing appropriate in game menus.
**Update 3**
A lot of people are having issues with the progression of this achievement tracking properly. Some people in the comments mentioned different things that seemed to cause the game to "catch up" with progression, if you will. From anything to recruiting another strike team, to actually playing an APEX mission in multiplayer. Keep in mind, just cuz it worked for others, doesn't mean it will be the fix for you. Just another thing worth trying. Unfortunately it just seems that the tracking on this achievement is pretty buggy at the moment.
**Update 4**
Courtesy of a few commenters it looks as though checking your status via the prestige section and making sure to debrief number 25, and then access the prestige page again to have it start back at 0/25 is making the achievement pop. Could be another random way to force progression, but multiple people are reporting this method works.
You start out with 1 Strike Team to send on missions with the ability to recruit more. The first time you recruit it's free, then your second team costs 40 mission points, with each proceeding team increasing by 40 points. (0, 40, 80, 120, 160, 200)
When you choose your missions, some of them will give you the ability to send a Strike Team or play in multiplayer. Do whichever you'd like. Completing 5 will net you:
I believe, given the wording on this trophy, that by 'Successfully Complete', it means that you have to win the mission and not fail it, 25 times. I will try to confirm that however unless someone else can.
You can also send your strike teams using the Apex HQ app which can be downloaded for iOS or Android. I can confirm that you can send out your strike teams via the app and it will count. You will not gain the trophy from the app, you will first have to go online via the game and log in from the Tempest, at which point the trophy should unlock for you.
Edit: In the Multiplayer menu under Prestige -> Challenges -> Accomplishments, you can check your progress towards this under the Veteran Accomplishment. You will see a #/25 and how many times you've completed it. Most people say that this does not unlock even though most have completed it. To date I have completed it twice and am at 23/25 towards completion number 3.
Edit 2: waited and made sure to unlock the 25th successful mission via the multiplayer hub in the game, not the app. As soon as I did the debrief, I got the trophy. It would seem that using the app to get the successful missions is fine as long as the final (25th) mission debrief occurs in game, not in app.
I only did 25 and it unlocked. Also, I failed a couple and it unlocked. Might be a different glitch now, but they've droped support so.
Just like with the "Unwavering" achievement, you have an option of getting this achievement done in two ways. To send a strike team out, you can either enter the MP menu (but instead of going in to start a co-op match, choose "Strike Teams" from the list of options), or do this from the command center on the Tempest. Choose an easy mission to start and send a team out. Bronze missions will take 1-3 hours, so check back and send them out again when they return. These missions can be "failed" and you won't get a +1 toward the achievement if that happens (and you run the risk of getting a permanent negative trait attached to the team which will make them less effective, so try to only send teams with a high percentage of success). Just be sure to check back often to keep starting new missions, and recruit more teams when possible to speed this up.
NOTE: You can also work on this from the "Apex HQ" mobile app. However, make sure you complete the 25th mission in-game for the achievement to unlock properly.
For the actual MP route, you need to complete 25 total co-op APEX missions by successfully extracting. These can all be done on Bronze difficulty if you wish.
You can do this trophy in single player or multiplayer.
In Single Player:
On the Tempest, you can deploy Strike Team on simulated mission. The teams will be gone for a couple hours, and will either complete or fail the mission. They get experience either way and level up constantly. Higher level teams have a higher success rate, but if you fail too many missions, the teams that failed might pick up a negative trait that hinders their ability. Complete 25 missions by sending Strike team out constantly to earn this trophy.
In Multiplayer:
You can complete 25 coop extraction missions. Each extraction mission consists of 7 survival waves while also having other objectives such as defending a point, disabling devices, or eliminating HVT's (High value targets). On the 7th wave, you have to survive for 2 mins and extract. Successfully complete 25 of these coop missions to earn the trophy.
Warning: if you are using the APEX app to send strike teams, make sure to send the last one while playing, as many have reported the trophy will not unlock unless you do so.
UPD: As of version 1.04, missions through the application do not count towards achievements
Explorer / Researcher" . So do everything in the mobile app, and then just get one success report in the game itself.
