How to unlock the Explorer achievement in Mass Effect: Andromeda - Definitive Guide
For this achievement player must complete 5 APEX missions.
APEX missions are a subset of strike team missions. Not all strike team missions are APEX missions. APEX missions are indicated by Andromeda symbol on them (icon). These missions - unlike regular strike team missions - can be played online OR player can send strike teams to complete them.
Achievement requiers to complete 5 of those missions - win or lose. A lot of people have trouble unlocking this achievement. I too was frustrated that the achievement didn't unlock for me when it should. The way I tried to do it was from Tempest deck in single player game. Strike missions can also be accessed from multiplayer menu. So I went there, started to browse through menus and when I opened APEX challenges section both achievements - this and Veteran - unlocked.
Note, that for me progress bar for this achievement on xbox one was at the 0% all the time - it never went up. Other gamers reported that this is also the case, so you shouldn't stress too much about it.
My guess is that you have to complete tutorial in order to start making progress on it / unlock it.
So you don't have to accept missions results on strike missions screen in order to unlock this achievement. It may be needed though because you have to trigger something in game menus which will check mission counter in order to unlock achievement (eg browse through APEX challenges in multiplayer menu).
Also, my suggestion is not to mix two methods of obtaining this achievement, as this may be a factor (eg: when you switch method, achievement counter gets reset to 0). But it may be a wild guess.

It unlocked for me right after playing my first multiplayer quick match game. I had sent tons of strike teams already, so I was confused. Perhaps you need to not only do the tutorial (I did it twice) but also play a round?

You unlock the ability to send in Strike Teams, after gaining access to the Tempest and speaking with Kandros on the Nexus.
You start out with 1 Strike Team to send on missions with the ability to recruit more. The first time you recruit it's free, then your second team costs 40 mission points, with each proceeding team increasing by 40 points. (0, 40, 80, 120, 160, 200)
When you choose your missions, some of them will give you the ability to send a Strike Team or play in multiplayer. Do whichever you'd like. Winning or losing will count towards your total number played. Completing 5 will net you this trophy.
You can also send your strike teams using the Apex HQ app which can be downloaded for iOS or Android. I can confirm that you can send out your strike teams via the app and it will count. You will not gain the trophy from the app, you will first have to go online via the game and log in from the Tempest, at which point the trophy should unlock for you.

ATTENTION! I completed all the tasks from the mobile application and completed about 30-35 of them, but they never gave me the trophy. Then I simply collected a report from one task in the game itself and I got 2 trophies at once - this one and "Veteran / Veteran" . So do everything in the mobile app, and then just get one success report in the game itself.
