ME2: Visit 100% of the planets in an unexplored cluster

How to unlock the Galactic Explorer achievement in Mass Effect Legendary Edition - Definitive Guide

This can be done in the first Cluster you are started in called the Omega Nebula. You don't need to fully deplete each planets resources, just launching 1 probe is enough. You can buy more probes and fuel at the fuel depot.

Edit: according to ExactBananas launching a probe is not required, only entering the orbit is. Also others have mentioned this not unlocking and being buggy. I'd suggest leaving 1 full system unexplored if this is happening for you that way if a patch fixes it, you can load your save and have one available to use to pop the achievement.

I have the same problem. I've explored 100% of the planets in every cluster in the game so far but the achievement hasn't popped.

This just unlocked me, and I didn't have to launch a probe to do it either. Just enter the orbit and that's enough for it to be considered 'explored'.

I can vouch for @ExactBananas comment - when I went into orbit around the last planet, the achievement popped for me.

I’d fully explored several systems and no luck popping this one until I got to Osun (Hourglass Nebula) and entered the orbit of Pergatory. Happy times!

You unlock this achievement by exploring every planet in a cluster, most of which include multiple systems with multiple planets. A planet is considered "explored", when you enter its orbit, not when it has been probed.

I've found that the most convenient way to do this is to wait until completing the main story mission Horizon which you unlock after completing the recruitment missions for Archangel, The Professor, and The Warlord.

After completing Horizon and opening the galaxy map, you'll find yourself in the Iera system in the Shadow Sea cluster, which wasn't available before completing the Horizon mission. The Shadow Sea only has a single system, Iera, which only four planets. Visiting all four should unlock the achievement.

If the achievement doesn't unlock you may have encountered a bug. Try closing the game and loading a previous save to try again. You can also try fully exploring a different cluster. As a last resort, a new playthrough may be required.

There are a number of clusters that can work for this. To raise the percentage of exploration, you simply need to enter the orbit of a planet in the cluster, and repeat for each planet. After recruiting Garrus, Mordin, and Grunt, you'll have access to the Shadow Sea cluster which only has four planets to explore making this one of the quickest by far.
