This is Thane's loyalty mission. Speak to him after recruiting him to your crew and he will ask you to go to the Citadel.
I played as a Paragorn as much as i could because thats how i saw Shepard, so i made all blue/Paragon/positive choices when i could, and in doing so won his loyalty.
You get the achievement after completing the mission.

Can you replay the mission if you failed it

i actually never tried to do that because i played the game when it first came out so i knew what to do already. there is a Restart mission option in the load/save menu you can hit to redo it but idk if that is still an option after you complete it. I would say to reload a previous save otherwise.

After recruiting thane at some point aboard the normandy you will get a prompt that he wishes to speak with you.
Travel to the citadel and have him in your party. Speak with bailey and ask about thanes son to initiate the mission
