I got this achievement by completing the mission - Kallini: Ardat-Yakshi Monastery. This quest originates as Mesana: Distress Signal, but when you go investigate the planet Lesuss in the Mesana system, new information turns the mission into a seek-and-destroy type of objective.
At the conclusion of the quest, outside the burning monastery, you will be faced with the choice of permitting the sole Ardat-Yakshi to live on or die.

at the end and the achievement popped. Unsure if you need to save
or just complete the quest.

I got distracted and didn't save Samara and the achievement still unlocks.

After the main quest "Priority: Citadel 2" speak to Liara to obtain this quest. It will be labeled as "Investigate Asari Colony" on the map. You can find a walkthrough for this mission HERE.
