you get this achievement after completing the Priority: Tuchanka mission.
If you have Wrex with you you will receive the Krogan support towards your endgame assets and Mordin will die.
If you dont have Wrex here they wont find a successful cure to the genophage and will not receive the krogan support.
(At least thats how it was in the 360 ME3. Unclear if they changed this for the new edition but i always just went for the cure for a more positive ending).

is finding the cure really the positive ending? seems like dooming the galaxy to me lol

Some of tasks (e.g. Citadel: Hanar Diplomat) are changed indeed in ME: Legendary. Instead of getting war assets for either of decisions, I've got all of them by stopping the transmission, which is different from x360 version.
