This is a missable achievement, but it's pretty hard not to get this at the end of the game if you played it normally.
If 2 or more squad mates have survived in the final mission, Shepard successfully makes the jump and will live.
If 1 or 0 squad mates have survived, Shepard doesn't make the jump and will die.
It's really difficult for this outcome to happen unless you haven't done any of the Loyalty missions.

For this achievement, you must have at least two teammates survive the suicide mission. Since you need the entire team to survive for No One Left Behind (75G), you'll get this as well. See that achievement for more info.
