How to unlock the Savior achievement in Mass Effect Legendary Edition - Definitive Guide

This is unlocked for finishing the Omega DLC.

You access it by talking to Aria T'Loak (in Purgatory on the Citadel) and then completing the story mission Priority: Palaven.

You will recieve a message from her on your private terminal titled "It's time"

From there, you will need to meet with her at Dock 42 on the Citadel and start the mission.
(As far as I know, you will be locked into completing it where you need to finish the whole thing before you can get back to the rest of the game)

do be careful this is like a 3-5+ hour dlc that you HAVE to finish once you start, i went into this around lvl 40 and came out around 46, still a great and fun dlc

Not missable. There wouldn’t be a way to miss paid dlc (in the original release).

Going to Dock 42 isn't quite the point of no return. You get a longish cutscene /conversation when you get there but you then go back to the Normandy and can do whatever other missions you want until you're ready to recommence the DLC quest, it only starts when you join Aria's fleet which is on the galaxy map located next to the Citadel.

Anyone got any idea why this might have said it’s unlocked in the in game achievement menu but not actually unlocked on my Xbox achievements?

This achievement is for completing the Omega "DLC" story mission. You can access this anytime after "Priority: Palaven" and will get the quest from Aria T'Loak in Purgatory (on the Citadel). Simply completing the story quest will unlock the achievement.
