This achievement is for mass effect 3. You can get it fairly early in the game.
As long as a system has the reaper icon on it you can goto the system, then press the scan button which is LT.
to alert the reapers your there. On my video they are automatically chasing me as i'd already been there are already alerted to me. Then a loud sound will happen, which is telling you that the reapers are chasing your little SR2 ship on the map. There is a bar at the bottom left telling you the alertness status.
After they appear and start going for your ship, quickly goto the mass relay, as soon as you hit A
on the mass relay the achievement will pop.
Best to make a save before you do it, just incase the reaper chased you down and you've been hit by it.
It's a very quick achievement to do.

For this achievement, you will want to enter a Reaper-controlled area (you'll see their ship over the system you enter). When you are inside the system, use 
to scan and once the Reaper Alterness bar is filled, a Reaper will appear and chase you. You need to escape through a Mass Relay once the Reaper appears to unlock this achievement. So long as you are scanning near the relay, it should be extremely easy to get to it and escape the system.
