How to unlock the Medal of Exploration achievement in Mass Effect - Definitive Guide
Simply land on ANY planet that can be landed on.
It doesn't matter what cluster or what planet, as long as you land on it with your Mako, you will get this achievement instantly.
Example: Edolus in Sparta in Artemis Tau. You can reach this planet as soon as traveling becomes available.

Can you name a planet that should work? I've tried several and haven't had any luck.

Once you have control of the galaxy map, land on a world that does not require you to go there in order to progress the story. The trophy will unlock once you land in the mako.

Might be useful to add to the guide that the meteor from the Bring Down The Sky mission also counts for this trophy, I just got it that way.

A very easy achievement, especially if you plan on getting any of the achievements requiring you to beat the majority of the game. A large amount of the side-quests in the game require you to go to uncharted planets, so you will get this achievement early on. As soon as you land on any uncharted planet, the achievement will unlock. You don't even have to do anything, just land in the Mako.
Note: Therum, the planet you will find Liara on, does not count towards this achievement. The planet you land on must be optional for the story.
