How to unlock the Medal of Honor achievement in Mass Effect - Definitive Guide
Finish the game on any difficulty to net this achievement.
I would recommend to start on normal, then start a new game on Hardcore when you are finished with your character from your normal game.
After completing the Hardcore run you will net 2 more achievements after completing the campaign again, one for completing the game on Hardcore and for doing 2 playthroughs.
Also, after finishing your 2nd run on Hardcore, you will unlock Insanity Difficulty
This will help reduce the playthroughs you need if you are going for completion.

It sucks ass that the game ends after you beat the story line, I was hoping to resume my character afterwards......

You can use your character again, unless u meant continuing the story after beating the game.

I think it needed to be added that Casual does indeed unlock this achievement. I got it today and did not get a reassuring link about it.
Finish the main quest. Now, I recommend you start out at Casual. No, Normal is NOT Hardcore, and Veteran is NOT Insane. So, go easy, find out where everything is at. Save your money for the next playthrough. Make sure to always use two allies and do all of the side quests with them.
