Checkpoint 5.
I had 2 painkillers at this point, don't know if its the same for everyone reloading this checkpoint.
There are a bunch of guys near cars, I would let myself get shot up and use the 2 painkillers then reload checkpoint until the achievement popped.
Hope this helps if you want it done quicker and if you have already done the story :)
