In Round 2-C, after the people in the bar chase you to lick cake off you, you'll escape through the bathroom window.
This puts you into a side scrolling beat em up, Streets of Rage type level. Keep proceeding to the right and eventually you'll come across a red sports car. Beat this up until it explodes and then press action on it to pop the achievement.

I never encountered any people trying to lick you until after this level as the bar level was initially locked. I just hit the old man and the Streets of Rage section started.
I think it depends which actions you perform because I didn't unlock the bar level until I stuck the dynamite in the cake in the party level.
