How to unlock the Smooth Operator achievement in Medal of Honor - Definitive Guide

At the End of the Mission First In You will encounter a scene very much like the final part of Mile High Club in Cod4, you will automatically change to your pistol, and you will see the hostage being held in the centre of the room. Aim at the hostage takers head and fire.

This achievement is obtained in single player mode in the first mission "First In". At the end of the mission, your team will clear a barn-like building and once you enter the room, the game will turn into a slow-mo effect similar to Call of Duty's 'breech' segments.

As you move into the room, you will see an enemy grab a tied up man center screen. Shoot the enemy with your equipped pistol in the head while your teammates clear the other enemies in the room, and the achievement will pop.

Near the end of the level, an objective “Clear Last Building” will appear on your screen and your team will be camped outside a door at this point and someone mentions “Go Soft”. This is the cue for an upcoming slow motion breach scene. As soon as you enter look straight ahead for a guy with a white headwrap who will reach for the hostage (in brown headwrap) and headshot him. When you have to clear the last building (you will get a checkpoint before, so you can restart last checkpoint if you mess up), you will kick down the door. It then goes into slow motion and you pull out your pistol. The guy you have to kill with a headshot will grab the hostage (he has a symbol above his head). Simply aim and shoot him in the forehead.

In the very first mission at the end, there will be a moment when entering the door you will see how the Taliban is taking a man hostage, you need to quickly kill the Taliban with a shot in the head from a pistol.
