Every village has many kinds of items laying around on the ground, on tables, in chests, etc. When you approach one, you'll be prompted to Steal by pressing 'E.' Do so in front of a villager, and this achievement will unlock.
Be warned, you will lose Dynasty Reputation if you get caught stealing - however, if you have the 10x Dynasty Reputation modifier active, it's an inconsequential amount, and there are no other repercussions. To add on, you can also drop stolen items and steal it again in front of NPCs for it to count if you want to get it quickly from a new game.

I'm not 100% sure about this, but it seems that all 50 times need to be within the same township. I stole 42 separate items in the starting town, and then went across the river to sell them. I stole 8 more items, getting a reputation decrease and no achievement. I dropped all of the stolen items, picking them up one by one (maybe a dozen in total), and after the 4th time got the achievement.
