I seriously struggled to unlock this at first.
Dead serious. Dying by hunger/thirst, fall damage, wild animals, bandits, all failed. I even tried dying as my heir by having him take over via dialogue.
I finally popped this by having my son be age 18, dying as the father, and getting a screen telling me my son was taking over.
Im unsure if this was intentional or if a recent update changed the requirements but if you too are stuck, try this method.

I feel the pain, same here!Did you turn Unlimited Health OFF when the son reached age 18?

I did, yes. After trying various times I kept it on till he could take over. After, took it back off.

Step One: SAVE YOUR GAME
Step Two: Pick a fight with something/someone (I ran up and punch one of those bison things)
Step Three: Die, collect achievement, reload save

This was my last achievement, i had to delete all saves to finally get it to pop. Then ran straight to some wolves in a new game. Just an FYI.

Hello, Did this today.
All i did was chop a tree down and as it was falling walked under it. Did this twice and i died.
