This achievement is story related and cannot be missed.
Once you obtain the Havoc Gloves during your second visit to the Industrial District, you have to clear the first puzzle to get back to Old Town.
It's the easiest of the Havoc Puzzles. There are two rocks that block your path and you need to approach them by moving upwards towards the rock, then move left to avoid being crushed.
At the top of the screen, push the next rock into the gap then push the next rock as far as it will go. Dig down then left and press A to exit the puzzle. The achievement should unlock as soon as you press A.
