This is based on how many stages you complete, not your highscore, so there's no benefit to going for it on the Medium/Hard defenses over the Easy defense.
As for strategy, just work on getting a general as soon as possible. I find myself losing at 18/19 if I haven't got generals by then.

so you can get to 20 on multiple levels or must all be done on the same level?
