Easiest way I have found Is In Mission R-01. Simply play through until you fight Mistral, she will have 4 dwarf gekkos with her at the start of the battle; they will all run at you in a pack so wait for them to get close to you then enter blade mode, one Horizontal Slash & BOOM! easy achievement.

No need to add a video for this achievement. One of the best spots to earn this is during mission R-01 when you are battling the Fenrir mini boss.
After you deal a third of his life bar of damage he will jump away and summon 3 humanoid cyborgs that all charge you at the same time. Parry the first attack and enter blade mode, take aim with a horizontal slash and all three should be pretty much bunched together every single time because of how they spawn.
If this doesn't work just restart checkpoint.

this achievement is really easy. There a couple of ways to do this actually. You can either have three people at low health and put them in a group or a line so that your blade can reach or you can get three people in a straight line and slide under their feet during ninja run and activate blade mode once your ass is scraping the floor. I managed to do this on one of the VR missions because the enemies have limited space and they always stick together so I recommend that you do that. Hope this has been helpful.

This achievement can be done earlier than during the LQ-84i-fight.
When you start R-01 (choose normal difficulty), the first battle consists of three cyborgs. When the fight begins, run at them with your Ninja-Run (RT) and then press Y to slide into them. While sliding, activate Blade Mode and adjust your cut so that you hit all three red boxes in a single hit. If you don't hit, load checkpoint and retry.

This is not as difficult as it appears, there are many opportunities where 3 Cyborgs will be in close proximity, you just need to enter Blade Mode and slash as Cyborgs are affected without weakening, this can be done easily in some VR Missions as you do not need to do this exclusively in Story mode however the battle against the Fenrir boss in R-01 summons 3 Cyborgs which you can easily perform this on. Additionally this can be performed on Dwarf Gekkos if you're struggling as these are always grouped up and chopped up instantly.

I assume that most people would get this fairly randomly throughout the game. Find a cluster of enemies, trigger Blade Mode and hack away.

This is as simple as it sounds, while in Blade Mode hit three enemies with one slash and have it dismember all of them. This is easy to perform on the basic cyborg as you do not have to weaken them and they tend to crowd around Raiden. It can also be performed against Dwarf Gekkos as they're small and can easily swarm you, giving the perfect opportunity to slice at least three of them at once.
