Like the tutorial says; when time slows down a little bit you can enter Blade Mode, allowing you to slice through enemies. You can manually angle the cut using right stick; flicking it, and pan using left stick. If you don't care to do that, you can press 'X' for horizontal attacks or 'Y' for vertical slices. It it worthy to note that it is faster to use 'X' and 'Y', but if you manage to sever a limb entirely (not into little pieces), it will yield extra points; which adds up over time.
Additionally, some enemies will not die from egregious maimage, so your best bet is to always aim for the red square.

It seems like if you cut and finish with a Zandatsu it doesn't count towards this?

As long as you are using blade more (R1) you'll earn this trophy as you play through the game. I earned this in the fourth stage (R:03). Some enemies will require weakening before you cut them apart, but if you use Zandatsu as much as possible, you'll quickly unlock this.
Kagerasimaru, Kheapathic
This is a very simple and straightforward trophy almost to the point of it being unmissable. While playing the game use Blade Mode or Ripper Mode to dismember enemies or merely slash them into ribbons. Keep in mind the more powerful enemies can't be dismembered until they've been weakened.

Chop up some enemies in Blade Mode. Should get this achievement fairly quickly.
