How to unlock the Tearing Away the Disguise achievement in METAL GEAR RISING: REVENGEANCE - Definitive Guide
Before you attempt this, you need to know that you must do this in a single playthrough without quitting and selecting a different mission. Doing so will void the achievement, as you will see in the video below. The video is just to demonstrate the Gekko locations and how to access them.
R-02 For this one you need to first clear the tunnel of all the Dwarf Gekkos. Once cleared head back to where you dropped down.
R-03 For this one you need to progress through the level until you reach the underground rail line. From here just follow the line to the end of the area where you need to exit. The Gekko is in the far right corner away from the exit door.
R-04 Head through the missions until you reach the final area. From here you will need to follow the path to the boss fight ahead. Before you fight, look to your left and you will need to use a rocket launcher or grenade to kill the Gekko.
R-05 This one can be a little tricky as you need to sneak through the area with this next Gekko. Make sure you have a box or barrel handy. Once you clear the second area, use the barrel or box to sneak past all the other Gekko's, then make your way down the stairs to the Humanoid Gekko below. You must not be seen during this progress, if you are, reload your checkpoint.
R-07 At the very start of the level, run across the road and you will find this one between two rocks.

So can I reloads checkpoints or save, exit to title and then resume without exiting the game?>

You can indeed. You can also resume and reload checkpoints. Just don't jump around missions using the chapter select.

There are a total of 5 of these in the story, they just look like 2 Dwarf Gekkos, one standing on top of the other.
Humanoid Dwarf Gekko #1 - R-02, This will be once you encounter a bunch of Dwarf Gekkos, once defeated backtrack and there will be a Humanoid Dwarf Gekko where you came from.
Humanoid Dwarf Gekko #2 - R-03, This will be once you reach the underground area, at the very back, beyond the exit will be this Humanoid Dwarf Gekko, you may need to use AR mode.
Humanoid Dwarf Gekko #3 - R-04, This will be once you approach the boss on the Helipad, before heading forward turn left and shoot this Humanoid Dwarf Gekko with an RPG/Stinger
Humanoid Dwarf Gekko #4 - R-05, This will be at a section where there's some large stairs, you will need a Box/Drum in order to hide from 2 Sliders, being detected will cause the Humanoid Dwarf Gekko to disappear to be sure to sneak close by and fire an RPG/Stinger to kill it.
Humanoid Dwarf Gekko #5 - R-07, This is right at the beginning, to the right of the road and at the back will be the final Humanoid Dwarf Gekko.

During the story there will be 5 pairs of dwarf gekkos stacked on top of one another apparently trying to be "human". Finding these guys, and taking them out, will net you this trophy.
However, unlike the collectables in this game, you need to get all 5 done in one go. For example, if you missed the second one, and go back to Chapter 3 to take him(them) out, the trophy will not pop. You need to get all 5 in order of appearance.
Below is a video by Somewhat Awesome Games that does a fantastic job displaying where each of these weird looking gekkos are. If you guys get a chance, give him a thumb's up on youtube.
Also note that the humanoid dwarf gekko in Chapter 5 is annoying. You have to get to him via stealthy means. The vide details how to get this done.

This achievement is fairly buggy. Community speculation has set some parameters for unlocking this achievement. In order to unlock it, you need to play from mission R02 to R07 without quiting out. I believe the achievement should unlock upon completion of the mission.
For a guide of where these are, please check the collectible guide.
(Glitched) This trophy requires you to find and destroy all 5 humanoid Dwarf Gekkos that are hidden in the game. Some are more easily found than others, but all can be easily missed. While not entirely missable, this trophy is finicky, with some people having to destroy all 5 in one pass and others being able to quit the game and pick it up later and still earn the trophy. This problem is made apparent, as unlike other collectible trophies, the game will not auto-save once you kill a humanoid Dwarf Gekko. For safety's sake, I would advise to destroy all 5 of them in one go without using the restart function.

The trophy is not cumulative, you need to kill everyone during the 1st playthrough.
+Be sure to make sure that the Gekko that is right in front of the Sundowner explodes first and only then disappears, because the kill may not be defended

I’ll add to all of the above that if you didn’t get the trophy on your first playthrough, then it’s not at all necessary to start a new playthrough of the campaign, as they say on many forums. Just select the very first chapter in Chapter Select and move strictly according to the plot; you cannot select chapters separately (but at least you will save your entire leveling, which will make replaying much easier).
If in the process you have personal matters that cannot be postponed, then leave the game without any problems - as soon as you return, you can safely select the Continue option, the trophy will still be counted, do not believe people who claim that the trophy can only be won if you go through the entire story at once , without turning it off (who could have even thought of this?.
The whole point is that you don’t start playing the dlc (if you have one) or don’t select a separate chapter through Chapter Select. Tested on myself - today I knocked out the trophy myself when replaying with all the wigs and improved hp and energy strips.
