I recommend doing this on Very Easy or Easy as The End's awareness increases with difficulty.
One important note is that just holding him up where a "!" shows above his head is not enough. Hold him up the first time and keep your aim on him. He will eventually drop the Moss Camo. Once this happens, the achievement will unlock.
As P00R GRENDEL points out, unless The End has a bead on you, it's nature that will give away your position. Mostly it will be birds getting rustled out of the tall grass as you crawl through. Also, try to take out The End's pet parrot first, as it will also hone in on you and call out your location. On Very Easy/Easy the parrot is at the very start of the boss battle, on a tree stump you can see just by turning around.

If you’re having difficulty sneaking up on him when you get close, I would recommend using the the D-pad and not the left stick. I was able to sneak up, hold him up and have him drop the Moss camo in Hard difficulty by walking up behind him using the D-pad.

I crawled towards him. If he doesn't know which direction you are coming from, it's almost impossible for him to see you unless you scare some birds into the air or something like that.
