If you complete all the missions required (Deja Vu is not necessary) 100% stealth with no kills (1 level being an exception), no reflex on, next to no checkpoint abuse in a relatively quick amount of time, then you will get the S rank.
I know that may sound like a lot, but the fact is this game is one map that, once effectively learned, can be a breeze to run through no matter what the objective. So learn the map - complete a few runs on the mission, understand the enemy patrols, and steer clear of high guard concentrated areas, and you will be fine.
The only one I had difficulty with was the Jamais Vu mission, and time was my enemy. I would take too long
I found with that mission you need to no more than 12 mins - if you are taking longer than that, then no matter how amazing you did with the rest of it, you would not get the S Rank.
For Ground Zeroes, I took a while just because I was trying to mark everyone. Ended up with an S Rank by accident. Just stayed stealth, rescued all 7 POW's, killed no one and kept it under 40 mins.
The Classified Intel Acquisition was the easiest for me, and you can get it done with no kills and stealth in no time at all. You can follow the very simple guide for this achievement.
The Intel Operative Rescue was much easier than I anticipated as well, as long as you go for the pacifist achievement, and do not abuse checkpoints.
Eliminate the Enemy Threat was the only mission where I just straight up killed people (the two marks in question). If you go for the depth achievement, and do well enough to get (I believe) a A rank, then you will have a sniper rifle from the mission start. I picked that up, and made a B-line to the road that leads out of the camp on the right. There is a guard standing in a booth - tranq him. Stand on top of the booth and you have both targets well within your sniping range. Pick them off, and then continue to the road to the right out of the area. Should take you ~2 minutes, and give you an S rank (despite the kills).
Destroy The Anti-Air Replacements I found to be a lot of fun. Take your time, remain unseen, kill no one, and go for the Rescue achievement, and you will be fine. I highly advise using the C4 for this mission, and remember: you only need to take out 3 of the guns, so focus on the ones that are not overly guarded. A lot of the enemies use the same patrol patterns as in the Ground Zeroes mission, so you should be familiar with what guards are stations where. I had a blast here, and kinda lost track of time, but even using an excessive amount netted me the S rank while saving all 4 POW's.
Just remember: this is a stealth game. If you run-n-gun, then you going to have a bad time. Remain unseen, kill no one (with Palitz and Glaz being the exceptions ), and (for most) try to make it snappy and you will get the S rank.
Also, as an end note: When I say "Do not abuse checkpoints", I mean just that. Hitting checkpoints in the double digits is ABUSE, and you will pay for it with the nasty negative points modifyer that hits you endgame. Understaning where the checkpoints are is helpful, but every time you use one it is -300 points to your total score. No reflex (most of the time) will offset that and then some, but if you are hitting those checkpoints like crazy (i.e. ABUSING THE CRAP OUT OF THEM) and you end up with a -3,000+, then it doesn't matter how awesome you were everywhere else: you won't get the S Rank.

After you have successfully finished a mission within the game, you will be presented with a stats board which awards you with points based off your playstyle - rescues performed, enemies killed, alerts raised, retries used - and the amount of time it took to complete the mission. For each of the six missions, their S-rank point criteria differs quite a bit and these are the rough estimates:
- Ground Zeroes: ~50,000+ Points
- Eliminate the Renegade Threat: ~45,000+ Points
- Intel Operative Rescue: ~20,000+ Points
- Classified Intel Acquisition: ~50,000+ Points
- Destroy the Anti-Air Emplacements: ~45,000+ Points
- Jamais Vu: ~22,000+ Points
For this achievement, you will need to earn an S-rank in each mission at least once; difficulty does not matter. In order to do this, you will need to play smart; always take into consideration your surroundings, your weapon choice, your CQC approach, etc. As a few general reminders, keep these five key tips in mind when playing each mission, and if you are successful in being a true stealth-like operative, you will walk away with an S-rank for each mission without a problem.

Familiarization
- Play each mission at least once in order to understand where you must go for each objective and how to complete these objectives - time is of the essence for S-ranks, so completing each level quickly is top priority.

Non-Lethal Takedowns:
- Ditch the Assault Rifle, or any other weapon for that matter, and stick to Snake's Wu Silent Pistol; it knocks enemies out, rather than killing them (killing grants massive penalties) just make sure to always hide bodies.

Avoid Detection
- Raising suspicions or getting caught will harm your score, and have the entire camp on high alert with guards closing in on you; stick to the shadows and refrain from being caught.

Rescue Prisoners
- Take a few extra minutes to rescuer prisoners - each successful rescue and extraction earns you nearly four thousand additional points, so it is in your best judgment to rescue as many as possible, as quick as possible, all without not getting caught.

Practice
- This cannot be stressed enough; restarting checkpoints, getting caught, killing guards all reduce your final score, so practice each mission several times so you can complete them without any slip-ups, even if they are minor.

Do your best to avoid killing and alerting any enemies. Play smart and use the Wu Silent Pistol when necessary. Take your time, line up the reticle, and go for those headshots so baddies will fall asleep instantaneously.
You can finish all six missions on normal difficulty with an S-rank and earn the achievement, you don't have to worry about doing this on hard.
Upon clearing a mission you’ll come across a results screen. Certain records will be displayed for that mission depending on your performance. For the most part, most missions require that you garner 50000 points or higher as your final score before rewarding you with the S-rank. The only exceptions here are the "Intel Operative Rescue" Side Op and the "Jamais Vu" Extra Op, as these are more action oriented missions (approximately 20000 points should net you the S-rank provided that you don’t kill many people while you’re out of the chopper in Intel Operative Rescue). These points you earn in the results screen at the end of a mission do not accumulate for repeated playthroughs of a mission; the points are only there for that most recent playthrough. Here’s what to keep in mind:
1. Be quick and efficient. If you have to replay a mission, keep the objective locations fresh in your mind and get it done. Don’t mess around, stick to the objective. Memorize the areas where guards stay put.
2. Enemy combat alerts. If you tranquilize someone with the Wu Silent Pistol, and a patrolling guard catches one of his allies "sleeping on the job," you should still be able to continue just fine as long as you aren't spotted. If that white detection line fills up on your screen and you enter reflex mode, take care of that enemy quick and kill only if you need to. Don't enter reflex mode too often if you have it enabled!
3. Killing subtracts from the final score you earn in a mission. If you kill one guard, that’s minus 200 points from the final score (if you kill two, minus 400 and so on). It's best to just use the Wu Silent Pistol.
4. Rescue prisoners. For each one you save, you are rewarded with 3500 points for your final score in that mission.
5. Do not lose your helicopter when you call one in. If enemies see it, they will open fire at it. Select a safe spot that is secluded from enemy units’ line of sight. I usually stick to the cliffside east-northeast of the cages where Chico is located in the main mission when designating a landing zone.
6. If you make a small blunder such as accidentally wasting one tranquilizer round, it won’t be worth it to retry a mission (unless you’re that stubborn). Restarting from the last saved checkpoint subtracts 300 points from the final score.
7. Disable reflex to earn a point bonus in the final score (not required though, I got S-ranks in all missions on normal with reflex enabled). It must be disabled before you start a mission to count.

I didn’t notice any difference in the game on normal and high difficulty, only that on high difficulty there are no lethal weapons (with a silencer) at the start, in principle, at rank S it is still advisable not to kill anyone (300 points are deducted for each killed person and 330 for replaying , 600 for an alarm - if I'm not mistaken), so there is no need for this weapon, and you can safely do it on high, and it’s much more interesting to play with a tranquilizer In almost all missions, additional cartridges for it are in the booth near the prison where Chico was rescued, if you go from the beginning of the first mission past the prison to the left along the fork, there is also a checkpoint that triggers (I didn’t have them there only in “Destruction of Air Defense”). To avoid wasting time, call a helicopter to evacuate prisoners, targets, or exit the mission in advance. You can drag all those rescued to one place, and then evacuate all together, but in the mission “Killing Traitors”, evacuate the first enemy separately from the second, because while you are chasing the second, the first will wake up only if you are not going to carry him with you.
In the Deja Vu mission, you only need to run fast to rank. To begin with, you should go through it collecting all the Events in order to get an idea of where, who and what is located. So, from the start we run at a gallop and turn right at the intersection, run to the tower near the helipad, there we carefully pass on the courts and focus on the perception of 2 soldiers and a helicopter, then past the wall - to the red door, break open the grate, turn off the lights - the memory is dark , and on the right side we run to the exit from the mission. 2 minutes and rank S.
