How to unlock the Insignia achievement in METAL GEAR SOLID V GROUND ZEROES - Definitive Guide

You need to find nine XOF patches, the video below is not mine.

One word of caution - the cheev doesn't pop until waaaaayyyy after the extraction.

After the extraction there're some cut scenes of:

and

Then the cheev pops.

EDIT: The first patch falls off your butt (seriously, it does) when you roll around a few times. To roll you need to get prone (Hold A), then aim your weapon (LT) and hold down the left stick to start rolling.

ATTENTION! For those people who collected all 9 badges and the achievement didnt' unlock, I solved this way: I just re-played the "ground zero mission" (on normal, I don't know if it counts) and after skipping al the cutscenes the achievement popped up

Just wanted to confirm that I got this after reinstalling the game and playing through again on normal, the difficulty I got the patches on. I let all the cut scenes play all the way through the second time, not sure if that made an impact or not though. Good luck to everyone and thanks for the help guys!

I thought this hadn't unlocked for me because it didn't pop up on my screen. I was sulking and looked at my phone and it had popped on there and was in my notifications on my Xbox so it's worth having a glance to see if it did the same for you. Good luck! Thanks for the guide!

The XOF Unit Patches are only available in the Mainstory Mission called "Gorund Zeroes". You must finish the Mission to get your Achievement. The Achievement unlocks really late after the last Cutscene respectively in the Rewardscreen.

The Guides above show the exact Location of each XOF Unit Patch and they are good but maybe you want to collect the XOF Unit Patches in your normal Playthrough?

Then i have a Video for you. My Video shows all Collectibles in the Ground Zeroes Mission, the Rescue Achievement and the S-Rank too.

Timeline for each Collectible: (The Map opens for each Collectibe, so you can quick located where I am)

01:50 - XOF Unit Patch #1
04:05 - Cassette Tape (Chicos Tape #5)
05:05 - Cassette Tape (Chicos Tape #2)
06:49 - XOF Unit Patch #2
08:31 - XOF Unit Patch #3 & Cassette Tape (MGS V GZ)
10:10 - Cassette Tape (Chicos Tape #3)
11:45 - XOF Unit Patch #4
13:50 - XOF Unit Patch #5
14:28 - XOF Unit Patch #6
17:30 - XOF Unit Patch #7

Excellent guide. Even though I stuffed up a bit at the end and ran out of tranq ammo (resulting in a combat alert), I still got an S rank though, so there's a bit of leeway with the score.

I take no credit for this solution. The 9 XOF patches afe detailed here http://m.uk.ign.com/wikis/metal-gear-solid-5-ground-zeroes/X.... There is a video included in the webpage for those of you who like to watch rather than read.

I would advise to set this achievement aside as a "lone unlock" as with all things, it is hard to collect all of them and still make a decent grade. For those who enjoy a text/picture guide, this should suffice http://www.ign.com/wikis/metal-gear-solid-5-ground-zeroes/X.... and for those who would prefer a video, here:
I do hope these help, enjoy and let me know if you have any suggestions in the comments!

XOF unit patches are a form of collectibles within the game that the enemy soldiers, as shown in the intro cutscene, have tossed away carelessly into Camp Omega while leaving via a helicopter. Thus, there are a total of 9 XOF unit patches that are scattered around the entirety of the map during the Ground Zeroes mission.

Most, if not all, of these patches are very well hidden. Add to this that the patches are incredibly small and are nearly impossible to spot outside of using night vision, and you will find yourself struggling to retrieve them all. It is highly recommended to follow the video below for a step-by-step guide in collecting these patches.

As a reminder, the achievement will not unlock right away upon picking up the last patch. Once all the cutscenes have played out after the mission – including the prelude cutscene to The Pantom Pain - the achievement is finally yours.

There are nine XOF unit patches. All of them are found within the Ground Zeroes main mission.

To earn credit for collecting any of the patches, you must finish the mission by extracting both Paz and Chico. If you happen to die before a checkpoint and you collected a few patches prior to reaching that checkpoint, you will need to collect the patches again. The achievement will unlock after gathering all nine patches once you complete the mission and get to the rewards screen after the cutscenes. You can skip the cutscenes if you've already seen them by pressing and choosing "skip."

Use night vision to help highlight the XOF patches if you're having trouble seeing them.

Pressing opens up a map of the camp. Make use of the compass there and direct yourself as needed.

Patch #1
From where you start the mission, turn around 180 degrees. Get to the edge of the cliff and move right, over to the bushes. The patch will be near there.

Patch #2
This is located in the same place where the prisoner is for the Rescue achievement. It's on the south-most side in this area, in front of the tents on a small pile of rock, sandbags, and garbage.

Patch #3
Head over to the cages where Chico is imprisoned and pick open the locked gate. The patch is on the ground directly in front of you, only a step or two from the gate.

Patch #4
From the previous patch, turn around and head over to the small shack/guard post. Climb up on top of it and pick up the patch.

Patch #5
There's a large building on the northwest side of Camp Omega. This is the Admin Building. Head over to the rightmost entryway (red-orange door), it's right by the huge gate that opens up to let in vehicles. Don't go through the red-orange door yet, but when you face the door, turn right and move behind the crates and cardboard boxes. By the blue mechanical device there's a drainage channel with the grate popped off. Hop in and the patch is right there, behind the bars.

Patch #6
From the previous patch, make your way over to the rightmost entrance of the Admin Building again. Go past the red-orange door. Head to the first white box-like structure that's roped off with chains and the little yellow posts. Jump on top of the container, leap to the roof on your left, and the patch is sitting on that little ventilation structure.

Patch #7
Outside the Admin Building is a helipad, and there's a windsock in the middle of the helipad. A few steps south-southeast of the windsock is the patch, sitting in a puddle of water.

Patch #8
Move southwest from the last patch, head past the chain linked fence, and you'll see three power boxes close together in another fenced enclosure near a set of warehouses. Pick open the gate, climb on top of the leftmost box, and the patch is there.

Patch #9
Roll on the ground for this one, you can do it anywhere. Hold to go prone. Then hold and push the right or left. Click down on the , and hold it down while pushing it right or left. You'll get a cutscene and the patch will dislodge from behind the pouch on Big Boss's ass.

If you have already collected, let’s say, 5 stripes and then the mission went wrong or something else, you can safely reboot, the collected stripes are saved.

To get the last patch (the one that Big Boss fell into his belt), you need to perform rolls on the ground: to do this, lie on the ground, hold L1, tilt the left stick to the left or right and press .The patch does not fall out immediately, but after 3-4 rolls!
