How to unlock the Accomplished achievement in Metal Gear Solid V: Ground Zeroes - Definitive Guide

You are not required to finish the missions on hard difficulty.

You will unlock the side ops by finishing ground zeroes for the first time.
You will unlock the extra op by collecting all XOF patches in ground zeroes.

You need only clear them, rank does not matter.

hasn't popped for me and I've done the required now. :( but I do have all the patches and that achievement. just this achievement hasn't popped yet

Popped for me today and I've never completed the last extra op "déjà vu" so that may not be required for this achievement

The Déjà Vu op was added in an update.The achievement applies to all of the other missions besides this one. It used to be a PS exclusive mission but isn't anymore.

You simply need to finish all missions once on normal for this. Below are my guides to finish the missions with an S-rank straight off and minimise the number of plays through each mission.

Within the game, there a total of six missions that must be completed before you can obtain this achievement. These are the main Ground Zeroes mission, four additional Side Ops, and the Extra Op, “Jamais Vu,” which unlocks once all the XOF unit patches have been collected.

By this point, the main mission will already be complete, as this is the only way to unlock the four Side Ops. With this in mind, all you have to do is complete the remaining five missions. Take note that all the missions take place in Camp Omega, so you shouldn’t find yourself too disoriented with the camp layout once you reach the third or fourth mission.

Each mission tasks Snake with completing a certain set of objectives, so tackle them whichever way you’d like. Once all six missions in the game are complete, the achievement will finally unlock.
