How to unlock the Unlocked achievement in Metal Gear Solid V: Ground Zeroes - Definitive Guide
First off, you don't need to play the Deja Vu extra op mission at all to get a single achievement in the game. It doesn't count towards progress on any of the achievements. You will need to complete the Jemais Vu extra op. for each step of this achievement.
So here is what you need to do to complete the "Unlocked" achievement.
1.Complete all missions on normal.
2.Complete all missions on hard.
3.Complete all missions on hard again, except for Intel Operative Rescue.
4.Complete all missions on normal expect to Intel Operative Rescue, making sure you mark all of the enemies with your binoculars or with iron sights. Every time you tag an enemy it will tell you how many enemies you have tagged so far.
That's it, those steps will unlock every trial needed for the achievement.
Thanks to MORDFUSTANG0 for the information regarding Intel Opertaive rescue not needing to be replayed. I'm not entirely sure if it needs to be played an hard, but most will be completing all levels on hard anyway for the "Information" achievement.
You need to complete each mission on both difficulties to unlock the first set of trials. You then need to replay 5 of the 6 missions on each difficulty again and complete at least one trial while doing so. Intel Operative Rescue only requires one play on each difficulty to unlock all the trials. For your 2nd run on hard it's always fastest completion time so you simply need to complete the stage again. Make sure to pick up any guns you've unlocked from beating them the first time to help speed it up a little. For normal it's mark every enemy on the map. Jamais Vu is easy enough with only 12 enemies.
The remaining 4 can cause some problems and take a fair amount of patience. I would suggest to remain in stealth as much as possible and try to only knock enemies out with the tranq. gun and not kill them as they'll just call in reinforcements the more guys you kill. Learn where all the auto-save points are so you can fall back and trigger them constantly. It's the same map for each mission with the same auto-save points so it shouldn't take long to learn them. If you trigger an enemy alarm you're probably better off just re-loading a checkpoint to avoid any more reinforcements. Sometimes it can bring the ones you need to you but I found it better to remain in stealth as much as possible.

You do not need to complete every trial - you just need to unlock them as the achievement says.
For example, completing the Ground Zeroes mission on Normal will unlock the "Fastest marking all enemies" trial. Completing that trial will unlock the "Fastest Neutr. of all Enemies" trial, but that is all you need: you do not need to complete the "Fastest Neutr. of all Enemies" trial at all, just unlock it.
And that is the same for every mission - your objective is to basically get all the trials unlocked (no question marks on any of the trial mission lists). If you do not hold a record in a trial, that is ok as long as you can see what the trial is. So you do not need to worry about in the Anti-Air Emplacement mission taking out the guns without weapons on hard, nor do you need to concern yourself with things like shooting all the Moai in the Intel Operative Rescue mission.
In essence, you will have somewhere around 10 - 12 trials to complete, and the trials on hard are basically replaying the mission for a "Fastest Completion Time", which is a freebie trial (just play the mission again). This is nowhere near as daunting of a task as completing all the trials, and because this is essentially one map, it is easy to figure out which path is the best for your respective objective after you have played a few missions and understood the layout.
Questions, corrections and neg vote justifications below.
6 Tips to make it easier and possible:
1- Before going for this achievement, do the one for obtaining "S" grade in all missions on Normal difficulty. That will help the second run a lot, where you have to mark all enemies because when you get S grade you unlock good weapons, specially the one that replaces the tranquilizer revolver for a damaging weapon with a silencer.
2- So, the idea for the "mark all enemies" missions is to mark 1, kill him right afterwards. I've read some people saying "Don't do that!", but there's no reason not to. In the worst case scenario you might end up having to search for the last guy, but then if it's just the two of you, you can attract attention without fearing getting reinforcements.
3- The idea with weapons with silencers is to kill enemies with just 1 shot, since the more you use it, the more the silencer gets worn and eventually will stop working. You can either kill enemies with 1 headshot in or out of the slow motion animation that happens when they see you.
4- For the Hard runs, by the time you go for them, you must be already pretty used to the single map this game offers and know your way around enemies. In the missions that I had to extract prisioners or get to the exit myself, I usually cleared a "line" for me to backtrack and run away later.
5- Don't try to cumulate the Hard reruns with any of the other achievements, so you can screw it (not be stealthy) at any time. Examples: in mission 1 on Hard I extracted Paz first, and then when it was Chico's turn I quietly took the enemies out just long enough to get to him. Once I had him I just ran to the extraction point, not caring if I got caught or not. On mission 2, same thing. Knowing where the bald white guy spawns I made my way there fighting as little enemies as possible, going all the way towards the prison, then passing the tower, etc. After killing him (the first target), I made my way into the building, aimed for the black guy without a military cap, then ran all the way to the exit, since for this second kill I went straight for the target, without taking care of his company first.
6- Also, when completing these challenges, do not bother to hide the bodies of those you kill. As long as you kill 1 at a time, they won't be replaced and so there's no consequence if they discover what you did once you're in another part of the base. Like I said before, just make sure you cleared out the guys that will stand on your way for whenever you come back running for your life.

You need to complete every mission on both Normal and Hard difficulty, with trials unlocking after when rewards are given out. Some will take multiple attempts. Marking enemies is best done from distance with binoculars or a scoped weapon.
Note: You DO NOT have to bother with Jamais Vu as the trophy doesn't count this level.

This is the most time consuming achievement in the game. From the way the trials are set up, you must complete a mission normally to unlock the first trial within that mission. You’ll then have to replay that mission, complete that first trial, and then finish the mission to unlock more trials. There are different trials on normal and hard, and you must play on both difficulties to unlock them all. Keep in mind that you must beat a mission on normal to gain access to hard difficulty for that mission.
The "marking" trial requirements can vary. For instance, once you begin the "fastest marking all enemies" trial in the Ground Zeroes main mission a notification will pop up on the left of your screen to track your progress (Marked Enemy [1/40]). While it does say 40 when you begin, certain enemies can leave Camp Omega, dropping the total. Also, when you alert enemies, backup units are called in, raising the amount of people you must tag to complete the trial. Make sure you get high up in those vantage points like the watch tower at the Admin Building, and mark people from there. Circle the entire map. Don't kill anyone prior to marking them.
As long as you play a mission on normal for the first time, go back to it again on normal to mark enemies (if it has that marking trial), play the mission on hard, then replay the mission on hard once more, the achievement should unlock. That's four playthroughs of each mission, two on normal and two on hard ("Intel Operative Rescue" only needs to be played once on normal and once on hard).
You can view trial records in the main menu. Locked trials will have ??? displayed and you won't see their names. Remember, you don't have to obtain a time or statistic in each trial, just make sure that you have no ??? showing up in your trial records.
If you read everything above and you're still confused, this is basically what you need to do. Be sure to complete the mission after you mark all of the enemies on normal. Fastest completion time on hard difficulty is simply setting your own personal best clear time. For "Intel Operative Rescue" all of the trials unlock after clearing it once on normal and once on hard. All of the other missions need to be played twice on normal and twice on hard.

This is the only achievement in the game which requires you to play each of the six missions on both Normal and Hard, because there are Normal trials, and Hard trials as well. The first trial of any mission - regardless of difficulty - will always unlock upon successful completion of that mission.

IMPORTANT NOTE - FOLLOW THIS RULE OF THUMB: Play the mission on Normal first, then play it again on Normal while marking all of the enemies, then play the mission a third time on Hard, and finally, play the mission on Hard a second time. PS It doesn't seem very simple, does it? In fact, everything is even more complicated than it seems
When completing the test "Quickly neutralize all enemies" - you cannot kill anyone, otherwise the test will not open and they will stand ??? (thanks to the hint above).
The same applies to “Quickly mark all enemies” because if you kill someone and don't mark them, you'll have to start over. However, it’s worth mentioning here that this can most likely only happen if you run through the mission like Rambo and especially use grenades and a rocket launcher, so I think there shouldn’t be any problems and this test will open automatically.
I've marked *(star) those challenges that will unlock automatically upon completion of the mission, which means you don't need to worry about them.
ATTENTION!
To open tests on normal difficulty where do you have ??? you need to complete one test completely (usually marking opponents) and only then will the second test open (it is no longer necessary to complete it)
To open trials on difficult difficulty, where you usually don’t have two trials open and instead have ???, you need to complete 1-2 trials (which you already have open, which means you will have to go through the mission more than once or twice) and then the remaining tests will appear (which are also not necessary to complete)
