How to unlock the Truth achievement in METAL GEAR SOLID V: THE PHANTOM PAIN - Definitive Guide

Real Requirements to unlock Mission 46
- Completed all yellow SideOps
- Completed all normal Missions (not the Extreme, Total Stealth etc ones)
- listened to all yellow cassette tapes (missed that, took me forever to figure that out. None of the Guides told me that)
- You don’t need to complete Mission 45, since you may don’t have Quiet as a buddy

Mission Tasks:
- Eliminated 6 soldiers at the hospital entrance
- Landed 20 or more shots on the man on fire

You probably ending up playing this mission four times (since your don’t know the Missions Tasks in the Prolog and now again), which is a pain – since this mission is so long. I hope you find the Mission Tasks her before starting this Mission.

I can also confirm that you have to start some mission after listening to all the tapes. When I was back in the helicopter the missions list was updated. Great guide, nevertheless!

Great guide :) I just wanted to add that if you for whatever reason dont get anymore yellow marked missions dont panic and instead chill out with some easy side ops. I did this and after a while I got a cutscene and got the next story (yellow marked mission)

Maybe they've changed the requirements but I've just done this mission and haven't listened to all the cassettes. Also would be good to say that you need to land the hits on the man on fire straight after you exit the hospital before he's hit by the ambulance. I was waiting until the shotgun section but you don't play that part in this mission.

You have to beat mission 46. I will not talk talk about the mission, but this will explain how to get it (or how I got it) as it's a little confusing.

To unlock this mission you have to do all the main missions up to mission 43. (you do NOT have to do mission 45)

You have to listen to all audio tapes that are yellow (just click on them you don't have to listen to the whole thing.)

You have to complete all yellow side ops.

You do NOT have to complete all side ops (I got it at 47 percent but I bet it could be lower) you do not have to complete any bonus mission (the extremes, total silences or anything else).

You may have to upgrade your mother base a little, but I had one part that was only at level 2, and I still got the mission.

After you finish mission 43, I'd recommend listening to all the audio tapes. Finishing any yellow side ops. Then play side ops, after 3-4 you'll unlock a cutscene back at the base. Play more side ops and you'll get a call unlocking the final mission.

If you're clearing 5 side ops with nothing happening there might be something I missed, but I completed them all (I did not have to do sideop 150 or start the process for mission 45)

Story-related, cannot be missed.

Similar to the achievement above, “TRUTH: The Man Who Sold The World” has several prerequisites that must be met before it is unlocked and ready to experience. Be sure to check them out below.

  • Complete all significant Side Ops (designated by yellow text).
  • Complete all "normal" main missions (Extreme, Total Stealth, & Subsistence variants do not count).
    • Mission 45, "A Quiet Exit," does not count either.
  • Listen to all the significant Cassette Tapes (designated by yellow text).

Once all of these pre-requisites have been met, mission 46, "TRUTH: The Man Who Sold The World," will be available in the iDroid’s mission selection list.

Story related. This mission is number 46 and is the last of the second chapter's story missions. In other words, once you've finished this mission, you've completed your first playthrough.

Complete all the main tasks and all the side tasks marked in yellow, then mission 46 will open for you, which will answer all your questions.

there are many guides for opening this mission, an approximate algorithm is as follows

1) you need to complete tasks 1-45, 47-50
2) listen/scroll through all the tapes with a yellow icon (helped)
3) complete all side quests marked with a yellow icon
4) constantly return to base to activate 'events'

some key missions (everything after 40 and beyond) open only after some time. THIS IS FINE. just complete 3-4 side quests, return to base and voila.

ps many people write that it is necessary to pump the database to the limit, but this is completely optional. got it like this. + many people write that you need to complete all previous tasks to rank s - complete nonsense. (but if you go for platinum, you get it anyway)

The mission appeared by itself. The completion rate is more than 60. All the cassettes were listened to. Side missions completed ~50%, i.e. didn't bother too much. Basic everything except repeated ones (extreme, autonomy, etc.)
