How to unlock the This Pounding Heart IV achievement in Metal: Hellsinger - Definitive Guide
On “Lamb” difficulty, you should be able to get this on Voke (the first level) using Paz (the skull).
Keep firing on the beat as soon as the level starts and you’ll keep your multiplier up, as well as getting into the perfect timing. If you take a couple of hits, no stress, just keep going.
If you need extra hits, destroying the health crystals count as one hit, as does destroying the small groups of pots that can be found around the level.
If you still struggling, the first boss at the end of the level is great for this too, as it can take a lot of damage and the projectiles it fires are relatively easy to dodge.
Here’s me doing this on Lamb, which you can tell because some of my shots are way out of time yet somehow doesn’t break the streak.

Can be done in the tutorial level very easily

If you're completely musically incompetent (like me!) or just frustrated with trying to get this, here's the absolute easiest way to get it: go to settings, accessibility, and turn on Rhythm Assist Mode.
Now, the only thing you have to focus on is not getting hit. Everything will be on beat, so you won't lose your chain unless you run out of enemies/objects or you get smacked by something.
I used Paz to make sure the enemies stuck around long enough for me to get to 50. Works for all the levels of this achievement.

This is absolutely the easiest way. I got it on the tutorial level within a few minutes. Thanks!

Easiest way I found to do this was on the first level, using the skull. It takes five hits to take down a marionette. There should be one to start with, then a group of four. Destroy the health crystal and move onto the next enemy. That should take you roughly into the forties. Above the cliff destroy the pots and the health crystal and take out the other two marionettes. That should take you to 50 hits no problem.
Here’s an additional strategy if you are struggling with this. The calibration was being a pain in the ass, and I couldn’t even get a 35 combo going, so I did this:
Get a metronome app (there should be plenty of free ones, or use a website), and set it to 110 BPM. That should perfectly match the beat on the first level, using the skull (Paz). Mute your TV! Hit start on the metronome and try to line it up with the arrows on the screen. Shoot to the beat. This should be easier to hear and keep in time. Practice at the beginning of the level and see if you can keep your “perfect“ streak. If not, stop and start the metronome until it seems good.
Now just go through the level and follow the tips in the other guides. I used the “streak guardian“ and “boon momentum“ sigils. You (probably) want to do lower damage so you can get more hits in without killing the enemies. I was able to get a 71 streak on my first try, just going around one room! Remember, it’s OK if you miss enemies. You only lose your streak if you get hit, or shoot/dash off beat.
Obviously this won’t help your aim, or help if you can’t keep time at all, but this was easier than wrestling with the calibration settings. Changing calibration mid-game did not seem to actually update it.

Easy method. Go to settings and at accessibility options scroll all the way to the bottom and select "always on beat". Just enter a level and start shooting without getting hit. Unlocks arent disabled in this mode. Best use the skull (paz) so enemies dont die as fast and you get more hits in.

Easily scored with the Groove in the first area of Vokk Hell, as well as the Heartbeat III trophy (Hit Streak: 35)

go through torture with the sigils “Rhythm Keeper 3” and “Impulse of Luck 3”, install them and you can get 50+ hits on easy in any location. The main thing is to avoid green flies, behemoth strikes and seraphim shooting beams.
