This achievement is glitched and for many players it hasn't unlocked as it should. Obviously this is achieved by completing all the challenges under the main missions, until you reach level 3 of each torment.
If that's not enough, you can't do anything other than replay the torments a second time since there should be a little problem with the synchronization of progress. If you are lucky you will be able to find the bugged torment in a few attempts, otherwise arm yourself with patience and replay the complete list following the order that the game suggests, to be sure not to skip any torment by mistake.

You absolutely do not need to get 3 skulls on all torments. I had played the first torment earlier in my time with the game, first getting 3 skulls and then at a later session restarting it numerous times to farm deaths. Then, today, I went in and played all of the tornemnts, starting with that one and progressing to the end. You only need 3 skulls in so much as you need that in order to unlock the later torments. The achievement popped when I got 2/3 skulls on my first attempt at Slaughter Mastery III.

This was glitchy for me but in a good way. It unlocked when I reached at least 1 skull in each of the challenges. I still had 4 I didn’t get 3 skulls in.

In fact only one skull is required for the achievement. Although it is necessary to get 3 skulls in the first two tiers of each torment simply to unlock the next tier.

Finished the PC version first and the achievement didn't pop. Had to replay the game on the Xbox finishing all the torments of a level before proceding to a new one, achievement finally popped up.

How the heck people beat this game for 6-8 hours, I played almost 9 hours and not even close. It’s very difficult to concentrate on a bit and enemies at the same time. I guess I just don’t have ear for music lol

Might just have to consider myself lucky, but I played 1 torment, went to bed, then played all the rest the next day in 1 sitting and didn't have to replay then other one I played in another session for it to pop. I shut down my xbox as well so I didn't utilise quick resume. I also didn't get 3 stars on all level.

mine unlocked after i only got a rank 2 on the final one. so i was like meh. don't need the skill haha, i think this achievement only needs rank 1 completed, as it doesn't specify, might makes sense why i got it with only 1 not being rank 3.

I had ALL the torments at 3 skulls on normal difficulty and it didn't unlock. I did them all over again (getting from 1 to 3 skulls and always hitting "continue" and never restart") and at the very last one it finally unlocked.

Over on Powerpyx it says that the Torments can glitch if you change difficulty setting, so that might be what's going wrong on the xbox version as well.

for the achievement you only need 1, i.e complete. devs are aware there is an issue. but they do think it's just the resume option. but they're looking into it

Still bugged. I lost a few hours redoing some torments, trying to remember which ones I could had restarted before. Too boring and frustrating. At least the solution helped me.

Thought I’d threw in how I just got it - like many, completed them all, no achievement. Restarted the game and my entire PC for good measure, booted it back up, and in one sitting got just one skull on all of them. When I got one skull I would just jump around until time ran out. It unlocked for my when I finished the second to last torment. I have no idea if one skull is necessary but I think I was super lucky in getting this

Glitched for me also - this worked to unglitch it: Completed all Torments again in a single sitting, in backwards order from last to first, getting at least 1 skull then letting the timer run out. It popped on completing the final Torment.

I did this in 2 sittings, restarted a couple before reading this, then continued after that - unlocked no issue.
Other version though had similar troubles, did them all 3 stars then ran through again getting 3's on all but four, unlocked after went back to the 1st one, getting 1 star.

You gotta get 3 pentacles (stars) on each of the first 18 to progress along. You only need 1 pentacle or better for the final three. I managed 3 on all of the first 18 and two of final ones (except Death’s Edge III), and I still got the achievement.

solution if it does not unlock, start the storms in phase order make from one to two skulls until the last storm of the last stage, you can choose to die or end the time to save the skull record

Glitched for me since launch, along with the 666 kills and 50 behemoth kills. Pretty sure my progress on these achievements was resetting every time I closed the game. Only managed to unlock them when doing it all in one go.

When I did this on both versions on Xbox, I left them all until the end after completing the main levels and just did them one after the other. I didn't move onto the next until I got 3 skulls. It popped no problem that way. Hope it helps.
alot of people unlocked this recently so going to assume the Dream of the Beast DLC/patch fixed this achievement

You dont need 3 skulls on every level, the last levels which is numbered 3 of the category, you can get whatever skull you like
And seems like it's patched I experience no problem with the achievement

Here to add a confirmation that 3 skulls are needed for all I and II torments. This is to unlock the III levels. Once you have everything unlocked, then you can simply earn one skull on each III torment and this will unlock.

unlike the xbox one version where i had to mess around and replay each torment (making sure to 3-star each again) and it eventually unlocked on a random one in the middle.. this one just popped.. but give the above a try if this one doesn't pop right away

Initially glitched for me. After I'd got the rest of the achievements I played through each of the torments again in one sitting, getting at least one skull on my first attempt of each and hitting "continue" after each reward screen. I got the achievement on completion of the 21st challenge. Someone else posted a solution saying he thought that hitting "restart" is what is causing some completions not to track, he might be right.

Can confirm you NEED ALL 3 skulls for almost all levels. The higher tier levels (ex: Relic Thief III ) requires you to get all skulls for previous tiers of that challenge otherwise it remains locked. Only on the final torments is it okay to get 1 skull. This means you still need almost all skulls from all torments :/
