How to unlock the Not Shaken, Nor Stirred achievement in Metal: Hellsinger - Definitive Guide
Update: As mentioned by @ChiflaGoodluck in the comment section this achievement is way easier than it used to be.
Go to options, Acessibility, and mark the last option which will make every move to stay on beat, this means you can play just like any other fps just kill stuff as you wish and you will be awarded with a 100% on beat when you finish the mission.
*This can also help with the 50streak achievement.

This achievement sounds ridiculous, however there is a very "easy" way to get this achievement without mastering the game. i only suggest that you finish the campaign and all the torments before trying this. Here is how i did it:
*First Choose The first Hell (VOKE) in Lamb Difficulty
*Then in the loadout screen you want to equip the Weapons : "The Hounds" and the "Hellcrow".
*For the Sigils you want: "Last Breath Aegis: III" and "Ultimate Sovereignty: III" both you can unlock doing the torments so make sure to complete them before going for this.
After that you are all set, all you have to do is play the level waiting for the Ultimate of your two weapons to charge thanks to "Ultimate Sovereignty: III", you will only use ultimates in the entire run, you can even do your ultimate off-beat and you will still get the achievement.
The strat that i used was just walking in circles around the area with the enemies while waiting for my ultimates to charge, i highly recommend that you dont try to dash because im pretty sure if you dont dash on beat the achievement will invalidate itself. So just keep walking/jumping and you are good.
The only time i actually play the game properly was to hit or dash towards the green crystals to get some health other than that there is no mistery just try to play safe waiting for the ults to charge. the one thing i strongly recommend is to only use the ultimate from your pistols, and stock the ultimate of the hellcrow just for the "Behemoth" (the big guys)
If you have the patience like i did you will get without any trouble in around 30minutes.

I am sooo impatient I can't just use the ult... but I never know if I actually missed a beat or not, wish there was a way to know within the UI if you are still at 100% or not. (i know you can tell with the streak loss, but if you get hit you also lose it so sometimes I am not certain).
Edit: Finally did it by clearing the first 4 combat zones with a few attack here and there and resisting not doing any attacks at the boss. this achiev took me the longest out of all of them.

i highly recommend that you dont try to dash because im pretty sure if you dont dash on beat the achievement will invalidate itself.
The achievement description specifically says “never hit off-beat”, not something along the lines of ‘don’t perform any action off-beat’, so you should be safe to dash off-beat.

If you struggle to get this achievement in the normal way, here's a tricky one. How can one never miss a beat? You guessed it right, if the one would never shoot.
Two weapons in the game have an ultimate that kills enemies without player's attention: Hounds that create a shooting clone, and Hellcrows that create a damaging Crowstorm. There's also "Ultimate Sovereignty: III" sigil, which charges your ultimate automatically, 1% per beat. That allows you to potentially clear the whole level without shooting once. You don't even have to use your ultimate on beat, the game would count ultimate kills as perfect ones in any case.
Enter the first Hell (Voke) on Lamb difficulty. I haven't tested if beating the level without literally shooting once would suffice, so you'd better kill the first enemy. Ignore the rest and head on to the first arena. Keep in mind to never use dash, because the achievement requires you to dash on beat, too, and there's no point in taking extra risks. Running and jumping is okay.
While at arena, keep on running in circles with Hellcrows. Once your ultimate is ready, use it and start running in the opposite direction. After the third arena wait for your Hounds ultimate to be ready and head on to the boss. Use the same tactics during the bossfight, just use Hounds instead of Hellcrows. Once you'll lower boss's HP to 50%, it would summon some enemies, which would require you to switch back to Hellcrows for a bit. After that no additional enemies would be summoned, so you can stop running and wait calmly behind the pillar, which would block all the boss's fireballs, while recharching you ult.
It can take up to 30 minutes to do the achievement that way, and, truth be told, that's extremely boring, but it's also fairly easy.

I can confirm you do need to hit the first enemy to have the on-beat counter start. Then begin the run with the ultimate cycle for the rest of the level.
If you just run through the level, like I did, you never started a counter so there was none to get off-beat.
