This means you must hit an enemy by 3 mines. Each mine do 4 damage.
So you need find an enemy above 8, and teleport it by using Ironlizard.
This could be easier in a boss battle, because boss has at least 50HP. So you can put 3 mines near the boss, then use Clark's Stargazer drop an enemy trigger the 3 mines. 3 mines hit the boss 12HP, and it popped.
