PSA: this achievement only unlocks with Clark's STARGAZER special action, which is the one that allows you to throw and move enemies. I unlocked the achievement by throwing an enemy to a neighboring square of one of my other units, who finished him off and the total damage was 12.

PSA: after some testing, I'm 100% certain this achievement only unlocks with Clark's STARGAZER special action, which may mean the description is just misleading or incomplete. I unlocked the achievement by throwing an enemy to a neighboring square of one of my other units, who finished him off and the total damage was 12.
I used Clark and Ralf, with Clark I threw an enemy soldier onto Ralf and he finished him off with his knife.

Use this method, cannot unlock.
I used Trevor and Eri's knifes, total damage 4+3+5, no cheevo.
Popped, Enemy's HP must above 10, I use 9HP enemy failed. But I popped by using 11HP enemy in higher difficulty.
