The video above works if your lucky and they spawn that way. For me they didn't spawn anything like what was seen in the video. The important thing to keep in mind is move quick and reload as you move. Your stepdad and Hunter will kill a few but you need to really hussle (even on easy). HEADSHOTS are key to this achievement!

This is a good guideline so that your not just spinning around aimlessly, but as mentioned before there is no guarentee they will spawn in any particular order as is mentioned in Undead Immortal's video. Don't know if this necessarily helps, just remember the video guide is not perfect so don't get frustrated like i did.
KEEP YOUR HEAD ON A SWIVEL AND STAY FROSTY!!!
"the Nosalises make a hell of a lot of noise running through the ventilation and will clue you to where they are." thanks to Beez 808

you might also mention that the Nosalises make a hell of a lot of noise running through the ventilation and will clue you to where they are.

Thanks! This was easier for me than watching a video.

Early in the Hunter level you'll be in a room with some other soldiers and the mutants will turn up having smelt blood from the hospital.
On Easy you can happily ignore any mutants that drop in from the big vent pipe at the top of the room. You only need to worry about the four ventilation grills.
I noticed that while their positions are random, they repeat if you reload your checkpoint. They are easy to kill and you won't need to kill more than five of them.

Just got that cheevo and I do confirm that if you reload checkpoint, the mutants come out of the same vents in the same order, Five kills worked for me.

Shortly after you meet the "Hunter" you will need to defend the wounded and sick from the Nosalises. Pick up the revolver on the right side of the room and shoot at any Nosalises that appear in the vents.

Take the revolver on the right side of the room, and shoot all the ghouls that appear in the ventilation holes. Watch the video

The achievement is not particularly difficult, the main thing is not to be distracted by the ghouls that will jump out of the upper shaft. 1-2 rounds are enough for one ghoul. I knocked it out easily on medium without straining.
