Beware: if you start new game plus on iron man mode and don't complete moscow before you quit out, you could lose your game save compeltely as I did.

Is it just me, but New Game Plus is not an option for me. If I highlight Slot #1, it says New Game Plus: Available, but when I select Play it just started a new game. I lost all of my collected weapons/armor. I played through again anyway for the alternate ending and now am back in the same position. Still no way to select New Game Plus.
Can you list out the explicit steps to launch it? It's possible the game is buggy after whatever patches have happened since.

This is for anyone who is unsure of how to access New Game Plus.
Beat the game. (I did it on Easy). Go to the main menu. Select the New Game Option. There should be New Slot and Slot #1. New Slot will say New Game Plus Unavailable while Slot #1 will say New Game Plus Available. Select Slot #1 and you'll get a warning about overwriting save game data. Select okay and you should see the option for New Game as well as New Game Plus. After selecting New Game Plus, you'll see the modification options. Select any you want and beat the game and you'll get the achievement. I selected Developer Commentary and Iron Mode as well as My Weapons at the bottom. (There is an achievement for beating the game on Iron Mode)
