To get this ending, you have to fuck up every "good" decision you made on the first playthrough. I have done at least the following things to get the bad ending.
(You will get a good point for instance at the Undercity level when you enter the first elevator, no matter what)
A negative moral point can be recognized by a little fading to black screen, or the Little One making a comment how sad he finds a bad decision, but accept it is human to make such decisions.
It can get a little confusing at times, since the loading screen shows another title than the level. Check your diaries. It should light up right where you are at the moment.
Reich:Kill the nazi at the end of the level who surrenders right after you got through the door after the chase and you step inside the train cart.
*Separation: Kill the man reading a book in bed who surrenders after you passed a bunch of guards.
Facility: Kill the nazi in the meat locker who surrenders.
Torchlight: Leave your lamp out during the lift ride.
When you get out of the elevator, leave your flashlight out. Don't help Pavel when he's pinned down by the spider.

Dead City: Kill both the Demon and its baby, will give you two negative moral points.
Red Square: Shoot and hit a Watchmen climbing and hanging on the Cathedral.
Leave Pavel for the souls instead of grabbing him (It's the part where Pavel begs for his life)
The Garden: Leave the mother mutant bear to the Watchmen and don't intervene when she is overpowered by the Watchmen.
There are some more bad choices but these are the ones that give you the bad ending achievement 100%. When Artyom can throw over the switch at the end of the game you have done it right!
After completing the story with the Redemption Ending: Load the Depot chapter and continue with the story, but make the following changes:
1. Kill Lesnitsky
2. Avoid all of the optional visions in Dead City
3. Kill the Demon and her baby on the rooftop in the Dead City
4. Let Pavel die - just stand still during the sequence
5. In the Garden shoot at the enemies while running to where the Bear fight is - I got 2 negative points: one before the long path towards the tunnel; and another near the tunnel entrance
6. After the Bear fight and it runs away, let the Watchmen attach/kill her -- I got the negative point while progressing
This was done on Survival and not Spartan - still took a little bit over an hour as you can now race though and stealth kill everyone
Do the complete opposite of Redemption, kill everyone and don't spare the two villains you come across. Don't give money to the poor or commit any positive actions. When you lose moral points your screen will fade a little and an ominous tone will play.
This achievement is obtained for destroying the D6 facility at the end of the Campaign. If you did not obtain enough Moral Points for the "Redemption" ending, this ending will automatically occur when Artyom reaches for the detonator via Miller's command.
To ensure this achievement will unlock without any issues, simply follow the suggestions detailed below:
- Disobey the Little Dark One and kill any mutants that you are told to leave alone
- Kill Lesnitsky and Pavel
- Kill the surrendering enemies in "Separation," "Facility," and "Revolution"
- In "Dead City," do not pursue the three optional visions
- In "Dead City," kill the winged demon and her newborns in the nest when you reach the rooftop building
- In "Red Square," kill everything possible – mutants and human snipers alike
- In "Garden," allow the mutants to kill the bear
If you follow the suggestions detailed above, you will have done enough "evil deeds" throughout the game in order to naturally obtain this ending/achievement. Finally, it must be stressed that this achievement should be paired with the Spartan Mode playthrough, due to the vast amount of killing that will occur between both human and mutant enemies and the deliberate deviation of stealth tactics during combat. The achievement will unlock as soon the the concluding cutscene begins to play.

You can find out what the ending will be at the time of departure from Polis to D-6 . Black says the phrase:
- “ If you lose, will it be bad for everyone? Then... "
- " ...I'll ask them to help " - this means that a good ending awaits you.
- “ ...I must save my own. You are good, Artyom. "- in this case you will have a bad ending .
- Steal (it is considered bad if you steal next to the owner, that is, you can take from boxes and safes at friendly stations).
- Kill;
- Do not spare those who have given up and are begging for their lives;
- Always move in a straight line, do not look for hidden places;
- Don't help anyone with anything.
