In total, there are seven visions in the “Dead City” level that count towards this achievement. Fortunately, four of these will occur through natural progression of the level while the remaining three will only occur after reaching a specific location off the main path. Utilize the checklist and video below for the exact location of these occurrences:

Vision #1 - Courtyard Playground
- This first vision occurs through natural story progression

Vision #2 – Piano Couple
- After reaching the second courtyard park, head inside the apartment building on your right, up the staircase and once inside the bedroom, the vision will occur

Vision #3 – Car Crash
- This third vision occurs through natural story progression

Vision #4 – Bathing Woman
- After the third vision, head towards the building directly on the left, and into head into the bedroom on the first floor where a vision of a woman taking a shower will occur

Vision #5 – Window Watching
- This fifth vision occurs through natural story progression

Vision #6 – Crying Toddler
- After dealing with the Demon on the rooftop, head down the staircase and into a side bedroom on the left where a small child can be found crying

Vision #7 – Subway Ride
- This final vision occurs through natural story progression
Once all seven visions have occurred during the course of your playthrough, the achievement will finally be yours.

Once all seven visions have occurred during the course of your playthrough, the achievement will finally be yours.
In the DEAD CITY Chapter, you will have to trigger 7 visions. Some are story related and some are missable. You also need all 7 in one playthrough. Here's a video guide by PowerPyx and a text guide for people who want that:
1. The first vision is of a playground and is unmissable.
2. Once a baby starts crying, a piano will start playing, follow the sound of the piano up the stairs to trigger the vision.
3. The third vision is of a car crashing into you and is unmissable.
4. After the car crash, climb the stairs of the nearby building to trigger a vision of a lady in the shower.
5. The fifth vision is of a family watching the missiles land and is unmissable.
6. After you encounter the demon on the roof, go down some stairs and halfway down is a little kid alone crying.
7. The final vision is of yourself in the metro as a young boy and is unmissable.
